ORDER FORM - this forms provides us with the necessary information to beginn the appraisal process. SRIP FORM - Small residential income property form is needed to complete the operating income statement and rent survey forms WAVIER FORM - this form is necessary to bill the appraisal fee thru escrow. PUD FORM (condominium)- to be filled out by those living in Plannded Urban Developments or in condominium projects.
